When will I receive an invitation from Farnham Dene Medical Practice to book my COVID Booster?

COVID Booster Vaccinations are given in accordance to the current Government guidelines which currently state boosters can be administered 3 months after the second COVID vaccination.

The primary way to receive an invitation from Farnham Dene Medical Practice is via text message and you will receive a message to book for your Booster at the Farnham Local Vaccination Service at Farnham Hospital. Please wait to receive this message to book your appointment. You may also receive an invitation from the National Booking Service to book at a local hub such as Fleet.

If you have a fever, please do not attend for your COVID Vaccination. Please rebook when you are fully recovered.

My COVID Vaccination details are incorrect on the NHS App

If you received your vaccination through the practice, please submit an eConsult using the Administrative function with details of your vaccination, including a photo of your vaccination card and one of the team will be in touch within 14 days

If you received your vaccination from another provider, please contact 119.

I received my COVID Vaccination outside of England and my vaccinations do not show on the NHS App

The NHS App is currently unable to support vaccinations given outside of England

I have had both my COVID Vaccinations and/or my Booster but it does not show on the NHS App

I am due to travel overseas shortly and need my COVID NHS Pass

You will need to register for Online Services with Farnham Dene Medical Practice using the form on our website.  Once we have received this form and verified your ID, we can link your online app to the practice. You will then be able to view your COVID NHS Pass. 

I have the NHS App but am unable see details of my COVID Vaccination or my full medical records

You will need to register for Online Services with Farnham Dene Medical Practice using the form on our website. Once we have received this form we can update your settings.

I have Patient Access but am unable to an NHS COVID Pass and QR Code

Patient Access provides your COVID Vaccination Status. If you require a QR Code please download and use the NHS App or contact 119.

I wish to view my child’s vaccination history through my NHS App

You will need to request Proxy Access in order to view your child’s medical records. Please visit our website for the form Farnham Dene Medical Practice and return the form to the practice via email to online.farnhamdene@nhs.net. Please note that children aged 11 and over are required to sign the form.
